Your Role

• Provide compassionate, personalized nursing care aligned with physician orders and care plans.

• Collaborate closely with the RN Case Manager and interdisciplinary team to ensure holistic patient care.

• Monitor and report on patients’ physical, emotional, and social health.

• Maintain clear and accurate documentation and communicate updates with patients, families, and the care team.

What You Bring

• Active LPN or LVN license in Arizona.

• Excellent communication, organization, and time management skills.

• Reliable transportation, valid driver’s license, and auto insurance.

• Current BLS certification (or willingness to obtain).

• Fingerprint Clearance Card (or willingness to obtain).

• MUST speak Spanish
