Your Role:

• Provide personalized, compassionate care in alignment with care plans and physician orders

• Work closely with our Nurse Case Manager and interdisciplinary care team

• Communicate clearly with patients, families, physicians, and staff

• Monitor and report on physical, emotional, and social health factors

What You Bring:

• Active LPN/LVN license in Arizona

• Excellent communication and organizational skills

• Insured, reliable vehicle and valid driver's license

• Current BLS certification

• Home Health experience and bilingual (English/Spanish) preferred

• Fingerprint Clearance Card (or willingness to obtain)
