Key Responsibilities:

• Plan and Direct Care: Organize and oversee home care services to meet the specific needs of each patient.
• Conduct Assessments: Perform thorough assessments of patients’ healthcare requirements.
• Create Care Plans: Develop personalized care plans in collaboration with other healthcare professionals.
• Provide Education: Offer counseling and education to patients and their families, ensuring they understand healthcare instructions.
• Communicate Effectively: Document clinical notes and communicate patient conditions to primary physicians and the care team.

What We’re Looking For:

• Licensure: Valid RN License with the Arizona State Board of Nursing.
• Experience: At least one year of experience in home health care or hospice preferred.
• Transportation: Valid Arizona driver’s license and reliable transportation.
• Certifications: Current CPR certification and a negative TB test (we can assist you with this).
• Background Clearance: Satisfactory fingerprint clearance per federal and state regulations.
• Bilingual (Preferred): Conversational Spanish skills are a plus.

Education:

• Associate’s Degree in Nursing or higher education required.
