Position Summary
We are seeking an experienced QA/Coding Specialist to join our clinical quality team. This role is responsible for reviewing clinical documentation, verifying proper coding, ensuring regulatory compliance, and supporting our clinicians in delivering documentation that accurately reflects patient needs and services provided.
The ideal candidate has a strong understanding of home health and hospice regulations, OASIS, ICD-10 coding, and documentation best practices.
Key Responsibilities
- Review home health and/or hospice documentation for accuracy, completeness, and regulatory compliance.
- Perform ICD-10 coding for diagnoses, procedures, and services according to industry guidelines.
- Audit OASIS assessments and provide feedback to improve data quality and alignment with patient conditions.
- Identify documentation gaps and collaborate with clinicians to resolve discrepancies.
- Monitor trends in coding or documentation issues and recommend process improvements.
- Assist with preparing for internal and external audits, surveys, and compliance reviews.
- Support ongoing staff education related to documentation, coding, and quality standards.
- Ensure consistency with Medicare, CMS, and state-specific regulations.
Qualifications
Required:
- Minimum 2 years of experience in home health and/or hospice QA, coding, or clinical documentation review.
- Strong knowledge of ICD-10 coding and home health/hospice regulatory requirements.
- Experience with OASIS review and quality standards.
- Excellent attention to detail and strong analytical skills.
- Ability to collaborate with clinical staff and communicate findings effectively.
- Proficiency with EMR systems (e.g., WellSky, HCHB, KanTime, or other industry platforms).
- Coding certification (HCS-D, HCS-O, ICD-10).
- Experience working in a fast-paced, team-oriented environment.
