Your Role

• Evaluate patients’ physical abilities and develop individualized treatment plans.

• Implement therapy programs focused on strength, balance, and mobility improvement.

• Educate patients and caregivers to promote independence and safe recovery.

• Collaborate with nurses, therapists, and physicians to ensure coordinated, holistic care.

• Maintain thorough and timely clinical documentation for all patient interactions.

What You Bring

• Active Physical Therapist (PT) license in the State of Arizona.

• Current CPR certification and valid driver’s license with reliable transportation.

• Strong communication, assessment, and critical-thinking skills.

• Prior home health experience preferred — new graduates with passion and initiative are encouraged to apply!
