Your Role:

• Evaluate patients’ physical function and mobility in their homes.

• Develop and implement personalized treatment plans to improve strength, balance, and independence.

• Provide patient and caregiver education to support safe and effective care.

• Collaborate with nurses, therapists, and physicians to ensure coordinated treatment.

• Maintain accurate and timely clinical documentation.

What You Need:

• Active Physical Therapist (PT) license in the State of Arizona.

• CPR certification and valid driver’s license with reliable transportation.

• Excellent communication, assessment, and problem-solving skills.

• Home health experience preferred — but we welcome new graduates eager to learn!
