We’re seeking a Home Health Clinical Manager Assistant to partner with our Clinical Manager in ensuring smooth daily operations, regulatory compliance, and the highest quality of care for the patients we serve.

This role is a key bridge between leadership and staff—helping coordinate schedules, monitor documentation, and track quality metrics to keep our team running at its best.

Key Responsibilities

• Support the Clinical Manager in overseeing and coordinating daily administrative and clinical functions.

• Monitor quality measures such as documentation completion, visit frequencies, and patient care outcomes.

• Assist with data collection, reporting, and quality improvement initiatives.

• Collaborate on staffing needs, including on-call, weekend, and per diem schedules.

• Participate in staff retention and training efforts.

• Track and report incidents, medication errors, and complaints as needed.

• Ensure confidentiality and compliance with all regulations and organizational policies.

• Consistently model professionalism, positive communication, and ethical conduct.

Qualifications

• Licensed Practical Nurse (LPN) license required.

• Minimum of 2 years of home health or hospice experience strongly preferred.

• Prior supervisory or leadership experience preferred.

• Excellent written and verbal communication skills.

• Strong organizational and problem-solving abilities.

• Ability to work effectively with patients, families, physicians, and staff from diverse backgrounds.
