What You’ll Do:

• Deliver personalized, compassionate care based on physician orders and care plans

• Collaborate closely with our Nurse Case Manager and interdisciplinary team

• Maintain strong, clear communication with families, physicians, and team members

• Track and report physical, emotional, and social patient needs with care and accuracy

Who You Are:

• Licensed LPN/LVN in Arizona

• Compassionate, organized, and a great communicator

• Insured driver with a reliable vehicle

• BLS Certified (current)

• Home Health experience and bilingual (English/Spanish) skills? That’s a strong plus

• Fingerprint Clearance Card or willingness to obtain one
