Your Key Responsibilities

• Patient Assessment – Conduct comprehensive evaluations to determine patients’ home care needs.
• Care Planning – Develop and implement personalized care plans tailored to each patient’s unique situation.
• Team Coordination – Lead and guide care teams, ensuring alignment with patient goals and needs.
• Counseling – Provide education and guidance to patients and their families to support effective healthcare decisions.
• Documentation – Maintain accurate clinical records and communicate updates to primary physicians.

What We’re Looking For

• Licensure – Active and valid Arizona RN license.
• Experience – Minimum of one (1) year in home health care, with proficiency in OASIS documentation preferred.
• Transportation – Valid Arizona driver’s license and reliable transportation.
• Certifications – Current CPR certification and Arizona Fingerprint Card.
• Language Skills – Bilingual or conversational Spanish skills highly preferred.

Education Requirements

• Degree – Associate's Degree in Nursing or higher.
