Your Role & Responsibilities

As a Home Health Physical Therapist, you’ll bring positive change to patients by delivering in-home therapy that restores independence and quality of life.

• Design and implement personalized treatment plans to help patients achieve mobility goals.
• Guide patients through therapeutic exercises and recovery techniques.
• Educate patients and families on maintaining long-term mobility and wellness.
• Collaborate with physicians, nurses, and the care team to ensure coordinated, high-quality care.
