Your Responsibilities

• Conduct in-home health assessments and create personalized care plans.
• Administer medications, treatments, and wound care as prescribed.
• Provide patient and family education to support health and independence.
• Monitor patient progress and adjust care plans as needed.
• Supervise LPNs and Home Health Aides to ensure high-quality care.
• Collaborate in interdisciplinary meetings for comprehensive care delivery.

Qualifications

• Licensing: Active RN license in Arizona.
• Experience: Minimum 1 year of RN experience (home health, hospice, or critical care preferred).
• Certifications: CPR certification and valid driver’s license with reliable transportation.
• Skills: Strong communication, organization, and interpersonal abilities.
