Your Role & Responsibilities

• Perform in-home health assessments and develop tailored care plans.
• Administer medications, treatments, and wound care.
• Educate patients and families on maintaining health and independence.
• Monitor progress and update care plans as needed.
• Supervise LPNs and Home Health Aides.
• Participate in interdisciplinary meetings to ensure comprehensive care.

What We’re Looking For

• Licensing: Active RN license in Arizona.
• Experience: At least 1 year as an RN (home health, hospice, or critical care preferred).
• New Grads Welcome: Ambassador program available for new graduates.
• Certifications: CPR certification and valid driver’s license with reliable transportation.
• Skills: Strong communication, organization, and interpersonal skills.
