What You’ll Do

• Patient & Family Support – Assist with care plans and address the needs of patients and their families.
• Collaborative Care – Work closely with interdisciplinary teams to provide holistic, patient-centered care.
• Communication & Coordination – Maintain open communication with team members and external agencies to ensure seamless care delivery.
• Data Collection – Assess factors affecting patients and their families to enhance outcomes.

Requirements

• Current LPN/LVN license in Arizona.
• Valid driver’s license and insured vehicle.
• Current BLS certification and Fingerprint Clearance Card (or willingness to obtain).
• Compassionate and adaptable nursing skills.
• Preferred: Home health experience and bilingual in English/Spanish.
