Key Responsibilities

• Plan, organize, and direct home care services tailored to individual patient needs.
• Conduct thorough assessments of patients’ healthcare requirements.
• Develop and implement personalized care plans in collaboration with care teams.
• Counsel patients and family members, providing education and healthcare instructions.
• Prepare clinical notes and communicate patient conditions to primary physicians.

What We’re Looking For

• Licensure – Valid Arizona State Board of Nursing RN License.
• Experience – At least one year in home health care or hospice preferred.
• Transportation – Valid Arizona driver’s license and reliable transportation.
• Certifications – Current CPR certification and negative TB test (assistance available).
• Background Clearance – Satisfactory fingerprint clearance per federal/state regulations.
• Bilingual Preferred – Conversational Spanish skills highly desired.

Education

• Associate’s Degree in Nursing or higher.
