Your Role & Responsibilities:

• Direct Patient Care: Assist patients with activities of daily living, including bathing, dressing, and grooming, under the guidance of a licensed nurse.
• Vital Signs & Documentation: Observe and record vital signs, intake/output, and other crucial patient information to support healthcare planning.
• Mobility Support: Help with range of motion exercises, ambulation, and safe transfers.
• Emotional Support: Offer companionship and emotional support to patients and their families.
• Environment & Confidentiality: Maintain a clean, safe, and confidential environment for each patient.

What We’re Looking For:

• Active CNA Certification with experience in home health (preferred but not required)
• High School Diploma or equivalent
• Compassionate, communicative, and patient-centered
• Physical Ability: Able to lift up to 50 pounds
• Reliable Transportation and ability to travel locally
• Strong teamwork and independent work skills
