Dependable Health Services is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our growing Home Health team serving patients throughout Sahuarita, Green Valley, and surrounding areas.

This full-time opportunity is ideal for an LPN who enjoys working independently, building meaningful relationships with patients and families, and providing personalized nursing care in the comfort of patients' homes.

What You'll Do

• Provide skilled nursing visits in patients' homes

• Deliver nursing care according to physician orders and the established plan of care

• Monitor patients' conditions and report significant changes to the RN Case Manager, physician, and appropriate members of the care team

• Administer medications and treatments as ordered

• Perform wound care and other skilled nursing procedures within the LPN scope of practice

• Educate patients and caregivers on medications, disease management, safety, and home care techniques

• Reinforce patient care plans and goals

• Communicate and collaborate with RNs, physicians, therapists, and other members of the interdisciplinary team

• Complete accurate and timely clinical documentation

• Help patients safely maintain their health, independence, and quality of life at home

What We're Looking For

• Current Arizona LPN license in good standing

• Home Health experience preferred

• Strong clinical assessment and communication skills

• Excellent organizational and time-management abilities

• Ability to work independently in a field-based environment

• Valid driver's license

• Reliable transportation and current auto insurance

• Comfortable traveling throughout the Sahuarita and Green Valley service area
