Dependable Health Services is seeking an organized, compassionate Licensed Practical Nurse (LPN) – DCW Intake Coordinator to support patients receiving Direct Care Worker (DCW) services throughout the Tucson area.

This is a full-time, field-based position with per-visit compensation. You'll work directly with patients in their homes, completing new patient intakes, assessments, supervisory visits, and required documentation while helping coordinate the services patients need to remain safely and independently at home.

What You'll Do

• Complete initial intake visits for new patients requiring DCW services

• Perform patient assessments and gather information needed to initiate services

• Conduct required supervisory visits for patients receiving DCW care

• Evaluate patient needs and identify changes that may impact services or the plan of care

• Help ensure DCW services are meeting each patient's needs

• Communicate with patients, families, DCWs, case managers, and the clinical team

• Complete accurate and timely visit documentation

• Report changes in patient condition, safety concerns, or service needs to the appropriate team member

• Assist with coordinating services for new and existing DCW patients

• Support compliance with company policies and applicable documentation requirements

What We're Looking For

• Current Arizona LPN license in good standing

• Home health, home care, DCW, or community-based care experience preferred

• Strong assessment and documentation skills

• Excellent communication and organizational abilities

• Ability to work independently and manage a full-time visit schedule

• Comfortable traveling to patient homes throughout the Tucson service area

• Valid driver's license and reliable transportation

• Dependable, compassionate, and patient-focused
