Dependable Health Services in Nogales is seeking a dependable and organized Licensed Practical Nurse (LPN) – DCW Intake Coordinator to support patients receiving Direct Care Worker (DCW) services.

This is a full-time position with per-visit compensation. The LPN Intake Coordinator will work directly with patients in their homes, completing new patient intakes, assessments, supervisory visits, and required documentation to help ensure patients receive safe, appropriate, and reliable care.

What You’ll Do

• Complete initial intake visits for patients requiring DCW services

• Perform patient assessments and gather information needed to initiate services

• Conduct required supervisory visits for patients receiving DCW care

• Assess patient needs and identify changes that may affect services or the plan of care

• Ensure DCW services are meeting the needs of patients

• Communicate with patients, families, DCWs, case managers, and members of the clinical team

• Complete visit documentation accurately and within required timeframes

• Report changes in patient condition, safety concerns, or service needs to the appropriate team member

• Assist with coordinating services for new and existing DCW patients

• Help ensure documentation and services remain compliant with company and regulatory requirements

What We’re Looking For

• Current LPN license

• Home health, home care, DCW, or community-based care experience preferred

• Strong assessment and documentation skills

• Excellent communication and organizational skills

• Ability to work independently and manage a full-time visit schedule

• Comfortable traveling to patient homes throughout the service area

• Reliable transportation and valid driver’s license

• Dependable, compassionate, and patient-focused
