What You'll Do

• Provide personal care and assistance to patients in their homes

• Assist with bathing, grooming, dressing, and other activities of daily living

• Assist with mobility, transfers, and positioning

• Support patients with maintaining a safe and comfortable home environment

• Observe and report changes in a patient's condition to the supervising nurse

• Follow individualized plans of care

• Maintain accurate and timely documentation

• Provide compassionate companionship and emotional support

• Communicate professionally with patients, families, and members of the clinical team

What You'll Bring

• Current Arizona CNA certification

• Current CPR/BLS certification

• Home health experience preferred

• Reliable transportation and a valid driver's license

• Strong communication and organizational skills

• Dependability and the ability to manage a field-based schedule

• A compassionate, patient-centered approach to care
